  • List of medications, including vitamins and over the counter medications
 
  • Immunization record
 
  • Any changes to your personal of family health history
Services We Offer that are not covered by OHIP :

 

Many health care services are covered under OHIP.  However, some are not.  As per guidelines set out by the Ontario Medical Association, the following are some of the services not covered through OHIP and a fee is associated:

 

  • Prescription renewals without a visit

  • Medical/legal forms

  • Government forms

  • Certificates for the purpose of travel, immigration, employment, etc.

  • Driver’s/aviation medical exam and other licensing forms/cerificates

  • Insurance forms, reports and certificates, such as: 

    • Disability certificate 

    • Treatment plan 

    • Insurance medical examination reports

  • Skin tag removals

• Completion of work and school related      

   forms/notes, such as:

     ∙Sick or back-to-work notes

     ∙Admission to day care/schools/                  

      recreational

     ∙Employer requested reports                  

      or forms (e.g., form physicals)

• Copying medical records, reports and letters

   at the patient’s request 

• Transfer or summary of medical records at

   the patient’s request

• Travel medicine advise and the injection of

   travel vaccines
